Player and Parent/Guardian Contract
In order for all players and parents/guardians to be on the same page with the coaching staff, we have developed a contract that both of you need to read and discuss. The contract explains what the coaches expect from both the players and parents/guardians on and off the field. Please print out contract and sign/date the appropriate areas. Have your son hand it in to his respective head coach.

Helmet Warning Letter
Please print out this form, sign it and return it to your respective head coach. ALL players MUST have a signed helment warning before stepping foot on the practie field. This comes directly from Nassau BOCES Athletics as well as the Nassau County, Section VIII Football Association.

Parent/Guardian Information Sheet
In order to have the proper communication, I am asking for each parent/guardian's contact and emergency contact information. Please print out this form, fill it out and have your son hand it in to his respective head coach. Thanks for your cooperation in this manner.

Physical and Parent Permission Slip Reminder
This form is a reminder that all players need an updated physical and parent permission slip on file in the HS nurse's office. Both of these important documents are valid for one year. If your son needs one or both of these documents, please follow the directions as stated on the form.

Website Registration Form
Each player must register for the website In order to be placed on a roster and have access to certain areas of this website. Players should ask their parents/guardians for permission as they will also be registered to the site. If anyone is not allowed to register for the site, please let me know so I can discuss alternative options with your parents/guardians. Thanks!!!
